How to refund/exchange item from a fully discounted order?

Hello,

consider an edge case, where full order subtotal was discounted (a custom 100% discount was added, for example). How is then possible to exchange(refund) the item? The Exchange/refund Items button is not shown in the order detail screen in the Orders application. The customer should be able to exchange/refund the purchased item.

Yes, that's how it works. Our Orders app doesn't allow refunds on an order where the amount is zero (or where the amount not yet refunded is 0). That's not intrinsic to our system, it's just app level logic. I don't have the history but my guess is that's it looking at it from a payment point of view not a stock / inventory tracking point of view.
