Redirect Error causing Page can't be reached error.

Hi,
Since this past Monday the redirect URL for my application Order Display has changed and the redirect url now has (&packageName=UNKNOWN) at the end and it periodically causes the "PAGE CAN"T BE REACHED ERROR". I don't have any APK files loaded onto my apps.

Here is the whole url. Any help given would be greatly appreciated.
